The city clerk's office accepts and processes passport applications on behalf of the U.S. Department of State. Passports are needed for U.S. citizens planning international travel.

Passport Application Checklist

  • Completed Passport Application (DS-11)
    • Form DS-11 must be typed or completed in black ink. DO NOT sign the form.
    • Form DS-11 can be downloaded from the U.S. Department of State website.
  • Passport Photo
    • One 2"x2" color photo taken within the last six months. Front view with a white or off-white background. Passport photo services are not available at the Commerce City Civic Center.
  • Evidence of U.S. Citizenship
    • An original or certified copy of one of the following documents:
      • U.S. birth certificate (must include full names of parent(s)).
      • A Certificate of Naturalization or Certificate of Citizenship
      • An expired U.S. passport
      • A Consular Report of Birth Abroad
    • Your citizenship document will be submitted with the application and will be returned to you by mail.
  • Identification
    • The identification must be current and government issued, containing your signature and photograph or physical description:
      • A valid Driver's License or Identification Card issued in Colorado
      • A U.S. Passport
      • An official Military Identification card
      • A Certificate of Naturalization or Certificate of Citizenship
  • Additional Documents
    • Applicants who have changed their name must provide an official copy of the Marriage License or a Court Order.
    • Supporting documents are required in cases of adoption, sole custody of a minor, or guardianship of a minor.
  • Acceptable Forms of Payment
    • Payment to the U.S. Department of State (passport fee) must be made on check or money order.
    • Payment to the City of Commerce City (execution fee) can be made on check, money order, credit card, or cash.
    • These are two separate payments
  • Children Under 16 Years Old
    • Both parents or legal guardians and the child must appear in person. Please see the U.S. Department of State's website for allowable exceptions.
    • Both parents must present valid identification. 

New Passport Applications

New passport applications are accepted from 8:30 a.m. to Noon, Monday through Thursday. Obtain an application from the city clerk's office or bring a completed application with you. You must make an appointment to apply.

You must apply in person if:

  • You are applying for your first U.S. passport
  • You are under the age of 16
  • Your previous U.S. passport was issued when you were under the age of 16
  • Your previous U.S. passport was lost, stolen or damaged
  • Your previous U.S. passport was issued more than 15 years ago
  • Your name has changed since your U.S. passport was issued and you are unable to legally document your name change

Passport Fees

The passport fees are as follows:

  • Adults (16 years and older):
    • $130 – Book (check or money order only)
    • $30 – Card (check or money order only)
  • Youth (15 years and younger):
    • $100 – Book (check or money order only)
    • $15 - Card (check or money order only)

Processing fee of $35 per application (cash, check or charge)

Important Note: When arranging payment, the processing fee cannot be combined with the passport fee because these fees are paid to two separate entities.

Passport Photos

Photos are not available at the city clerk's office. Applicants will need to bring in their own photo to attach to the application.

Criteria for the photo is:

  • In color
  • 2 x 2 inches in size
  • Taken within the past 6 months, showing current appearance
  • Full face, front view
  • Have a neutral, non-exaggerated facial expression
  • On a plain white or off-white background
  • Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head
  • Taken in normal attire
    • Uniforms should not be worn in photographs (except religious attire)
    • A hat or headgear that obscures the hair or hairline should not be worn (except when worn for religious purposes)
    • If the applicant normally wears prescription glasses, a hearing device, wig, or similar articles, they should be worn in the photograph
    • Dark glasses or non-prescription glasses with tinted lenses are not acceptable unless the applicant needs them for medical reasons (an accompanying medical certificate may be required if the eyes are not clearly visible)
